Carers Hospital Liaison Worker

Broomfield Hospital, Chelmsford
The Hospital Liaison Worker is at Broomfield hospital, Mondays Fridays from 9.00am until 4.30pm, or at other times by prior arrangement.

The role is to support unpaid family carers when their cared-for (or sometimes themselves) are inpatients in the acute hospital to contribute towards a timely, safe discharge and to avoid unnecessary re-admissions.

The Hospital Liaison Worker will provide information, support and advocacy. They will make referrals to outside agencies, liaise with health/social care professionals, attend meetings or simply have a cup of tea to talk over options for discharge with carers.
